It's a new year and a new medical form is needed. Please go to: https://filestore.scouting.org/filestore/HealthSafety/pdf/680-001_AB.pdf and download the medical form. You can print off a copy, fill it out, and leave it in the cub scout cubby or email completed form(s) to Heather Endsley or Jen Santos. Per BSA rules, a new form is needed each year.
